(1) Email your child's name, age, their 2 teammates names, & their Team Name to:
 QUEEN OF HEARTS REGISTRATION

(2) Potomac Storm will email a confirmation OR a waitlisted message
and a copy of the required Release of Liability

(3) Make Payment ($15 Per Player)

PAYMENT OPTIONS

(A) CASH - Receipt Provided

(B) CHECK* - Payable to Potomac Storm
*A $35 NSF fee will be charged for any returned check

**Due to Integrity & Security Concerns, We Will No Longer Accept Venmo Payments**

(4) Print, sign, and bring the Release of Liability, included below, or email an unredacted, electronically signed copy to QUEEN OF HEARTS REGISTRATION

YOU WILL BE ASKED TO MOVE YOUR CAR

ABSOLUTELY NO OUTSIDE FOOD OR DRINKS ALLOWED INSIDE, PERIOD
YOU WILL BE ASKED TO TAKE IT OUTSIDE

PARENTS MAY BRING CHAIRS & BLEACHER SEATING IS AVAILABLE
